What is Long Term Care?

Long Term Care (LTC) refers to the many medical, social, and support services needed by individuals who, because of chronic illness or disability, need help in caring for themselves over an extended period of time. Clinically speaking, someone is considered chronically ill if they are unable to perform at least two activities of daily living for a period of at least 90 days and/or requires "substantial supervision" to protect themselves from threats of health and safety due to severe cognitive impairment.

Long term care includes both institutional care in a formal facility, community based services for persons of all ages, and home care provided by medical professionals and non-medical professionals.

Long term care also refers to the financing, organization, and delivery of a wide range of medical and human services to people who are severely disabled or limited in their functional capacities for relatively long or indefinite periods of time. In medical terms, long term care is chronic care with the aim of management, control of symptoms, and maintenance of function. Long term care also has a vast nonmedical scope, and many individuals requiring long-term care are not sick. Perhaps they have been injured or born with a developmental disability which limits their activities. Otherwise they may be perfectly healthy.
